The information we collect will be used only for the purposes of this survey.

All survey information will be posted as general statistics. No names will be given.

Answers to questions will be posted along with the name of the person who answered it. We will not post the name of anyone who indicates they do not want their name posted. A check box is provided on the survey for this purpose.
